We take Blue Cross/ Blue Shield, Nevada Care, The Beechstreet Networks, MCC, Full Medicaid, Health Plan of Nevada Medicaid and others. If your insurance plan is not on this list, please call the office with your health insurance information. We can call your insurance company and check your benefits before your appointment. Q: Where are you located? Our address is 2810 Jones Blvd. suite 3. We are located on the Southeast corner of Jones & Tara-- 3 blocks South of Sahara. Please check out our Contact Us page for a map to our office. Q: How soon can I schedule an appointment? A: It depends on the type of office visit. If you need to come in for something like a urinary tract infection, we make every attempt to get you into the office on the same day. If it is for a new patient annual exam/PAP smear that office visit takes more time, and may take as long as 2 - 3 days to schedule. But, please call the office to inquire. We may be able to schedule you an appointment on the same day. Q: How long do appointments usually take? A: Depending on your type of office visit, it may take 15 minutes to an hour. Please inquire when you schedule your appointment. Q: How much do appointments cost? A: Please call the office to inquire. This answer depends on your type of office visit. Q: What do I need to bring to my appointment? A: A picture ID (like your driver's license), and payment for your visit in the form of cash, Visa or Mastercard. We no longer accept checks. If you have medical records or test results from a previous PAP smear, mammogram and/ or ultrasound you can bring those too. They may be helpful. Q: What is the difference between a Certified Nurse-Midwife and an OB/GYN? A: Please visit our About Us section or our Links section for more information about Certified Nurse-Midwives. Q: How many babies has Martha delivered? A: Over 2000 Q: I have moved and my new doctor needs my medical records. How do I get them? A: You need to fill out a release form which you can then fax or mail to our office. This is a form with your signature that states that we can release your records to a third party. Without this form, it is against federal law to release your records to another doctor unless Martha has referred you to that doctor to coordinate your care (such as a referral to a radiology facility for a mammogram). Then we can fax the records to your new doctor. If you want copies of your records, you must fill out a release form and there will be a charge assessed of $0.60/page.
